ROLE: Care Coordinator

SALARY BAND: Trinity Band C (£21,596 - £22,821)


WORKING HOURS: 3x 11hr shifts p/w

8am to 8pm including weekends and public holidays

An exciting opportunity has arisen to join our WEOLCCS team as a Care Co-ordinator. Enjoy a role working within a team that makes a real difference.

The goal of the end of life care system in Wandsworth is to support patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital. This role plays a vital part.

The nature of the role will require you to use your initiative and will involve interactions with distressed and anxious patients, their carers and families.

About the role

The key tasks within this role will be:

  • To provide an effective administration service and be point of contact to our patients, their family, carers and other healthcare professionals

  • To establish and maintain good working relationships both internal and external

  • To speak and communicate while adjusting to each situation accordingly ensuring that sincerity, empathy and compassion are shown.

About You

The successful candidate will:

  • Have proven experience in administration and an intermediate level of IT knowledge and skills.

  • Possess excellent communication skills

  • Be proactive, resourceful and organised

  • Adopt the values of the hospice and demonstrate commitment to achieving the goals of the team and the organisation.

  • Have a positive attitude and work collaboratively with others within and across organisational boundaries
