Care Coordinator (£21,596 - £22,821)

2x Part Time Positions 28hrs p/w

Royal Trinity Hospice is developing a new team caseload approach to patient care called ‘Team Around the Patient’ (TAP) in response to increasing demand for specialist palliative care services. A team of clinical professionals including specialist palliative care nurses, physio/occupational therapists and a social worker/counsellor will together manage a shared caseload of patients.

The Care Coordinator is a critical role within the TAP team, providing a single point of contact for patients, their carers and family members, GPs and other clinicians involved in a patient’s care. The Care Coordinator is responsible for all administrative support to the TAP team and will create and maintain effective office systems that enhance both the patients’ and professionals’ experience of care.
