What's happening News Innovative one stop shop for people with progressive, life-limiting illnesses celebrates milestone Over 200 people with progressive, life-limiting illnesses living in Wandsworth have benefitted from an innovative new service that aims to ensure they have access to high quality care and support, in the right place and at the right time. Funded by Wandsworth CCG as a two-year pilot, the Wandsworth Care Coordination Centre is jointly provided by Trinity Hospice, Marie Curie and St George’s Healthcare NHS Foundation Trust.Before the service launched in February 2015, patients, carers and professionals faced contacting multiple different providers looking for care and support at home. In times of crisis, this could lead to distressed 999 calls and unnecessary admissions to hospital. Now patients, carers and professionals have one point of contact for all their care needs so they don’t need to go to different care providers separately.The Care Coordination Centre delivers a more joined up approach so people can get good quality care and support at home when they need it. By improving the planning and coordination of care services in Wandsworth, it helps healthcare professionals to spend less time on administrative tasks, and more time on clinical work. The Wandsworth Care Coordination Centre is open from 7am - 10.30pm Monday - Friday and 9am - 5pm on weekends and public holidays.
