Salary: AfC Band 3 £23,213 - £25,195

Hours: 37.5 hours (mix of days and shifts including weekends and public holidays, from 7am to 3pm and 2.30pm to 10.30pm)

We have an exciting opportunity to join our award-winning Wandsworth End of Life Care Coordination Service as a Community Healthcare Assistant. This role involves providing high-quality care to people known to Royal Trinity Hospice who require care due to physical illness and general frailty and who are approaching the end of their lives. This care is delivered to patients in a variety of settings.

Responsibilities of a Wandsworth End of Life Care Coordination Service Community Healthcare Assistant

  • To demonstrate warmth to patients and their families
  • To recognise when changes occur, notifying a Registered Nurse
  • To undertake personal care and light domestic duties in patients' homes
  • To establish excellent working relationships, both internally and externally

Skills and experience required

  • To be kind and caring and comfortable talking to people
  • Caring for people who are approaching the end of their lives in their own homes
  • To have a full UK driving license and ability to travel across Wandsworth
  • To have the ability to work as part of a team yet be comfortable working alone
  • To possess or be happy to work towards completing the Care Certificate
  • To be computer literate
