Salary: AfC Band 3 - banding points 6-12 £24,210 - £25,615

We need flexible and enthusiastic people to join Wandsworth’s End of Life Care Coordination Service based within the Community Services at Royal Trinity Hospice working with patients in their own homes who are approaching the end of their lives.

We have an exciting opportunity to join our Wandsworth End of Life Care Coordination Service as a Community Health Care Assistant (Royal Trinity Carer). This role involves providing high-quality care to people known to Royal Trinity Hospice who require care due to physical illness and general frailty and who are approaching the end of their lives. This care is delivered to patients in a variety of settings.

We’re looking for candidates who:

  • Have the ability to work as part of a team yet are comfortable working alone
  • Can demonstrate warmth to patients and their families
  • Are happy to undertake personal care and light domestic duties
  • Have a full driving license that permits you to drive in the UK
  • Can work a mix of days and shifts including weekends & public holidays, from 7am to 3pm and 2.30pm to 10.30pm.

Skills and experience required

  • To be kind and caring and comfortable talking to people
  • Caring for people who are approaching the end of their lives in their own homes
  • To have a full UK driving license and ability to travel across Wandsworth
  • To have the ability to work as part of a team yet be comfortable working alone
  • To possess or be happy to work towards completing the Care Certificate
  • To be computer literate
