Salary: AfC Band 3 £26,618 - £28,065 (inclusive of HCA) dependent on experience

Hours: 37.5 hours/week

We have an exciting opportunity for an exceptional individual to join our Wandsworth End of Life Care Coordination Service (WEoLCCS) as a Community Healthcare Assistant.

Responsibilities of a Community Healthcare Assistant

  • Demonstrate empathy and provide compassionate care and support to patients and those close to them treating each with dignity and respect
  • Carry out personal care to patients referred to WEoLCCS as part of a team of carers
  • Recognise changes in patients’ condition and notify the Registered Nurse in the service
  • Collect information to assist the team in planning care
  • Care for the body after death and support family and friends appropriately
  • Recognise the needs of grieving people including recognising potential bereavement risks and escalate appropriately
  • Gather information to support holistic assessments of patients and those close to them to support care planning

Skills and experience required

  • Knowledge and experience of the delivery of health and or social care
  • Awareness of the role of Trinity’s community palliative care service and the wider work of the hospice
  • An understanding of the challenges of working with people with life-limiting illnesses and those important to them
  • An understanding of the need to adapt care and respond appropriately to changes in the patient’s condition
  • Possession of the Care Certificate or be willing to complete within 6 months of appointment
  • Ability to undertake a range of routine assessment and interventions in a community setting but willing to carry out new interventions following appropriate training.
  • Excellent communication skills and confidence to communicate with patients, families, and professionals from all backgrounds
  • Able to use electronic databases, or willingness to learn to record information following patient related interactions
