Head of Wandsworth End of Life Care Coordination Service Band 8a Registered Nurse - AFC We’re looking for a dynamic and enthusiastic nurse to lead our award-winning Wandsworth End of Life Coordination Service (WEoLCCS). This role involves leadership of the highest standard to ensure outstanding care for patients (and those close to them) and embedding Trinity’s vision and values in the workforce to help them realise their potential. Responsibilities for Head of WELCCS Provide operational leadership to the Coordination Service staff Ensure those eligible for CHC fast-track funding have their care needs assessed and implemented efficiently, and to the highest of standards Promote the Coordination Service to all stakeholders Participate in external meetings (e.g. ECP/GSF/MDTs) Share expert knowledge in the practice of outstanding end of life care in the community setting Commit to continuous improvement – if it can be improved, we will take action Skills and experience required Up to date NMC registration. Senior experience, supported by relevant professional knowledge and skills. The ability to lead or manage with a flexible approach and proactive mindset. A passion for caring for adults with a life-limiting condition, with a real commitment to delivering the highest level of care at all times.
