Lead Nurse – Wandsworth End of Life Care Coordination Service

Salary: AfC Band 6 (£37,638 - £44,780 pa)

Hours: 37.5 hours per week

We’re looking for a dynamic and enthusiastic nurse to join our award-winning Wandsworth End of Life Coordination Service (WEoLCCS). The WEoLCCS provides a dedicated, specialised care coordination service and central point of contact for patients likely to be in their last year of life and those who care for them (formally and informally). Trinity makes a real difference to people approaching death and helps set the standard for care and coordination for those in the last year of life.

The Lead Nurse will ensure that patients’ care needs are assessed carefully, produce care plans that are meaningful, relevant and accessible to the team of Royal Trinity Carers, who will deliver the care you decide. You will train and support the Royal Trinity Carers to be the best they can be, with a dual focus on their development needs, and taking every reasonable measure to ensure every patient experiences the excellence we are known for.

Responsibilities of a Royal Trinity Hospice Lead Nurse – Wandsworth End of Life Care Coordination Service (WEoLCCS)

  • To act as a community nursing end of life champion and inspire others
  • To line-manage and lead the Royal Trinity Carers
  • Ensure those eligible for CHC fast-track funding have their care needs assessed and implemented efficiently, and to the highest of standards
  • Promote the Coordination Service to all stakeholders
  • Ensure care is planned to meet the unique and changing needs of the patients, their families and friends. Share expert knowledge in the practice of outstanding end of life care in the community setting
  • Commit to continuous improvement – if it can be improved, we will take action

Skills and experience required

  • Up to date NMC registration.
  • Ability to plan outstanding care for those approaching the end of their life
  • The ability to lead or manage with a flexible approach and proactive mindset.
  • A passion for caring for adults with a life-limiting condition, with a real commitment to delivering the highest level of care at all times.
  • Excellent communication skills and ability to work collaboratively with all levels within the organisation
