Palliative Care Assistant, Community Services

Salary: £24,210 - £25,615 (Inclusive of high-cost allowance)

Hours: 37.5 hours/week

We have an exciting opportunity for an exceptional individual to join our busy specialist community nursing team as a palliative care assistant.

Responsibilities of a Royal Trinity Hospice Palliative Care Assistant

  • To provide responsive and timely holistic palliative care support for patients with an advanced, progressive, life-limiting condition under the guidance and direction from a Trinity registered professional
  • Carry out patient and carer reviews using agreed clinical outcome measures, to support development of a management plan as part of the multidisciplinary team.
  • Support patients’ friends, family, carers and other professionals involved in their care at all stages of the illness including bereavement.
  • Work alongside patients' usual care team to provide specialist palliative care and support wherever the patient calls home using a variety of methods, such as face to face consultations, telephone or virtual support using teleconferencing.

Skills and experience required

  • A good understanding of the needs of patients with life limiting illnesses and those close to them.
  • Experience within the health or social care sector is desirable.
  • Evidence of completion of care certificate or willingness to undertake.
  • Understands the role of Trinity’s community palliative care service and the wider work of the hospice.
  • Understands the challenges of working with people with life-limiting illnesses and those important to them
  • Understands the importance of and confident to participate in multidisciplinary team discussions
