Job Title: Palliative Care Assistant 

Salary: AfC Band 3 £24,938 - £26,385 (inclusive of HCA) dependent on experience

Hours: 37.5 hours/week

We have an exciting opportunity for an exceptional individual to join our busy community service as a Palliative Care Assistant.

 Responsibilities of a Palliative Care Assistant

  • Be the eyes and ears for the Registered Professionals who hold responsibility for the patient's care.
  • Use excellent communication and listening skills to enhance patients' psychological wellbeing
  • Gather information to support holistic assessments for patients and those important to enable the team to plan care accordingly
  • Work with Trinity clinical professionals to provide specialist palliative care, support, and rehab, this may include joint or independent community visits
  • Advocate for the patient and those important to them within multidisciplinary team discussions
  • Recognise when patients' conditions change and report this to the relevant member of the specialist MDT.
  • Recognise the needs of grieving people including identifying bereavement risks and respond and refer appropriately

Skills and experience required

  • Knowledge and experience of the delivery of health and or social care 
  • Awareness of the role of Trinity’s community palliative care service and the wider work of the hospice
  • An understanding of the challenges of working with people with life-limiting illnesses and those important to them
  • Possession of the Care Certificate or be willing to complete within 6 months of appointment
  • Ability to undertake a range of routine assessment and interventions in a community setting but willing to carry out new interventions following appropriate training.
  • Excellent communication skills and confidence to communicate with patients, families and professionals from all backgrounds
