Salary: Agenda for Change Band 6 £40,448 - £47,965 pa or Band 7 £49,036 - £55,049 pa dependent on experience (includes HCA Supplement)

We are looking for an experienced, enthusiastic and highly motivated individual to take up the post of Palliative Care CNS or Band 6 development clinical nurse specialist within our friendly and supportive community service. 

The Band 7 CNS post is suited to a senior nurse with significant experience of caring for patients with life limiting illnesses who possesses advanced clinical, teaching and communication skills. 

The Band 6 Development CNS position is an exciting opportunity for an exceptional nurse to join our community service at band 6 level and gain skills and experience on the job whilst providing senior nursing support working as part of a friendly, supportive, and dynamic team.

You will be a key member of the specialist palliative care MDT working across Royal Trinity Hospice’s community catchment area supporting health and social care teams in the delivery of palliative and end of life care wherever they call home.

Responsibilities of a Royal Trinity Hospice Palliative Care Nurse Specialist

  • Critically analyse complex clinical data and information to inform diagnosis and decision-making.
  • Deliver expert evidence-based patient-centred advice, care and support working in close collaboration with other health and social care professionals.
  • Communicate effectively with patients, their carers and all relevant agencies involved in the care of the patient to enable effective coordination of care.
  • Support clinical colleagues in decision-making in complex care situations to ensure appropriate care planning through the inter-disciplinary team meetings (IDT)
  • Regularly teach about the palliative care needs and management of patients at both internal and external training sessions.

 Skills and experience required

  • Registered Nurse with bachelor’s degree level training in palliative/end of life care or related subject.
  • Impeccable assessment skills.
  • Advanced communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
