Palliative Care Dementia Nurse Specialist

Salary: Agenda for Change Band 7 (£44,347 - £50,549 including HCA Supplement)

Hours: Full time (37.5 hours/week)

Royal Trinity Hospice is a leading provider of skilled, compassionate palliative care.

We have an exciting opportunity to join for a Palliative Care Dementia Nurse Specialist to join our friendly and supportive Community Services team.  We are looking for a nurse with extensive experience within the field of specialist palliative care, and a passion for caring for people with dementia.

Responsibilities of a Royal Trinity Hospice Palliative Care Dementia Nurse Specialist

  • Supporting people with dementia to engage in advance care planning.
  • Assessing the palliative and end of life care needs of patients with advanced dementia and their families.
  • Working closely with community health and social care agencies to coordinate and support the palliative care support for patients referred with dementia at the end of life.
  • Regularly teaching about the palliative care needs and management of patients with dementia at both internal and external training sessions

Skills and experience required

  • Registered Nurse with bachelor’s degree level training in palliative/end of life care, dementia or related subject.
  • Impeccable assessment skills.
  • Advanced communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
