Royal Trinity Carer Salary: AfC Band 3 £24,938 - £26,385 (inclusive of HCA) dependent on experience Hours: 37.5 hours/week We have an exciting opportunity for an exceptional individual to join our Wandsworth End of Life Care Coordination Service (WEoLCCS) as a Community Health and Personal Care Assistant. Responsibilities of a Royal Trinity Carer Demonstrate empathy and provide compassionate care and support to patients and those close to them treating each with dignity and respect Carry out personal care to patients referred to WEoLCCS as part of a team of carers Recognise changes in patients’ condition and notify the Registered Nurse in the service Collect information to assist the team in planning care Care for the body after death and support family and friends appropriately Recognise the needs of grieving people including recognising potential bereavement risks and escalate appropriately Gather information to support holistic assessments of patients and those close to them to support care planning Skills and experience required Knowledge and experience of the delivery of health and or social care Awareness of the role of Trinity’s community palliative care service and the wider work of the hospice An understanding of the challenges of working with people with life-limiting illnesses and those important to them An understanding of the need to adapt care and respond appropriately to changes in the patient’s condition Possession of the Care Certificate or be willing to complete within 6 months of appointment Ability to undertake a range of routine assessment and interventions in a community setting but willing to carry out new interventions following appropriate training.
