Staff Nurse - Community Services

Salary: AfC Band 5 - banding points 16-23 (£29,888 - £36,738pa)

Hours: 37.5 hours

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Staff Nurse working in the community. This post is suited to a nurse with some experience of community nursing and caring for patients with life-limiting illness approaching end of life.


You will be a key member of the specialist palliative care MDT working across Royal Trinity Hospice’s community catchment area supporting health and social care teams in the delivery of palliative and end of life care wherever the patient calls home.

Responsibilities of a Royal Trinity Hospice Staff Nurse - Community Services:

  • To provide responsive and timely holistic palliative care, for patients with an advanced, progressive, life-limiting condition when symptoms cannot be managed by their usual care team
  • Carry out holistic assessments of palliative care needs for patients and their families using agreed clinical outcome measures, identifying symptoms, and contributing to developing a management plan as part of the multidisciplinary team.
  • Support the patients’ friends, family, carers and other professionals involved in their care at all stages of the illness including bereavement.
  • Work closely with the Royal Trinity Carers (RTC), Continuing Health Care and the Care Agencies to ensure care is planned to meet the unique and changing needs of the patients, their families, and friends.
  • Build and maintain professional relationships both internal and external and to promote Trinity’s services

Skills and experience required

  • Registered Nurse with additional training or experience in palliative/end of life care or related subject.
  • Impeccable assessment skills.
  • good communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
