Team Around the Patient (TAP) in Kensington & Chelsea If you live in the Royal Borough of Kensington and Chelsea, from October 2018 you will be under the care of Trinity's Team Around the Patient, or "TAP". TAP provides specialist community services to you and your family in the comfort of your own home. More information about TAP for professionals What is the Team Around the Patient (TAP)? The TAP is a group of professionals who will deliver care to you in your home. We hope that this pilot project will mean that patients like you, your carers and family will have an improved, more seamless experience of our community care. The TAP includes specialist nurses, an occupational therapist and physiotherapist, a counsellor/social worker, healthcare assistants and care coordinators. They are supported by consultants and doctors in training (all supervised by consultants) who can visit you at home or see you in an outpatient clinic. The team will liaise with your GP, District Nurse and other services throughout our involvement in your care. The TAP works together to understand your needs, so they can deliver care to help you best cope with the impact of your illness. This means that the team member visiting may vary according to your individual care needs. The TAP can support you in many ways and can visit you during the daytime Monday to Friday. Download the information booklet about Trinity's Team Around the Patient If you have been a Trinity patient for a while (before October 2018) The care you receive from the TAP will be very similar to what you are used to, but you might notice the following differences: You will now have access to physiotherapy at home - patients under the care of the TAP have access to additional support. Talk to a member of the team during a visit about what this might mean for your care. The person who comes to see you might change - the TAP is made up of professionals with different expertise who share responsibility for your care. The decision about who visits you will be made by the team based on the main purpose of their visit and your needs at that moment in time.
