Since October 2018, we have been piloting a new approach to community palliative care in our catchment area in the south areas of Kensington and Chelsea and in July 2019, we extended the pilot into our catchment in the south of Westminster.

The Team Around the Patient or “TAP” pilot will shift our current model of community care, instead using a multidisciplinary team sharing a single caseload to deliver care. Our aim is to ensure patients and their families have a more seamless experience and help future-proof and adapt to accommodate growing demand and changes to our population’s needs.

The dedicated TAP team will consist of the following professionals:

  • Clinical Nurse Specialists in Palliative Care
  • Physiotherapist
  • Social Worker/Counsellor
  • Occupational Therapist
  • Care Coordinators
  • Palliative Care Assistants

Our out of hours service, inpatient unit and availability of medical staff to patients and the TAP will not change. The single point of access to the TAP remains the same: 020 7787 1062.

The pilot will assess how effective the TAP model is compared to our existing approach. It will run until November 2019 before being externally evaluated. The evaluation will incorporate feedback from professionals in primary care and community services.
