Salary: Trinity Band C (£21,596 - £22,821)

An exciting opportunity has arisen to join our Wandsworth End of Life Care Coordination Service (WEoLCCS) team as a Care Coordinator.

The goal of the end of life care system in Wandsworth is to support patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital. This role plays a vital part in that.

The nature of the role will require you to use your initiative and will involve interactions with distressed and anxious patients, their carers and families.

Working hours will entail 150 hours per four week period worked in thirteen 12 hour shifts with an hour unpaid break to be worked from 8am to 8pm and one 7 hour shift with a half an hour unpaid break to be worked to suit the team’s and the individual’s needs. These hours are inclusive of weekends and public holidays.

The key tasks within this role will be:

  • to provide an effective administration service and be a point of contact to our patients, their families, carers and other healthcare professionals
  • to establish and maintain good working relationships both internally and externally
  • to speak and communicate while adjusting to each situation accordingly ensuring that sincerity, empathy and compassion are shown

The successful candidate will:

  • have experience in administration and an intermediate level of IT knowledge and skills
  • possess excellent communication and speaking skills
  • be proactive, resourceful and organised
  • adopt the values of the hospice and demonstrate commitment to achieving the goals of the team and the organisation
  • have a positive attitude and work collaboratively with others within and across organisational boundaries
  • demonstrate personal resilience
