Triage Clinical Nurse Specialist

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Triage Clinical Nurse Specialist within our First contact team. This post requires you to act as the first point of clinical contact for the patient, following patient referral. They will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.
