Eligibility and the referral process Eligibility for care from Trinity This page contains information for patients and potential patients, their families and friends. We support people aged 18 or over who have a progressive, life-limiting condition and live in our catchment area. Our catchment area covers the whole of the London borough of Wandsworth, and parts of Lambeth, Merton, Westminster, Richmond upon Thames, Kensington and Chelsea and Hammersmith and Fulham. Our new Living Well outpatient programme of support, care, and activities is for people living in their own homes or other locations within the community such as care homes, nursing homes and temporary accommodation. Living Well may serve as an introduction to other Trinity services and palliative care support. We welcome referrals from patients who are frail, elderly, with a life-limiting condition or reaching the end of life. Please note, that this criteria is different to our existing nursing and medical criteria. You can see if you live within our catchment area by searching our interactive map. The teal/green area denotes our catchment for Community Services, Inpatient Care, and Living Well Outpatient care. The smaller purple area indicates our Inpatient Care and Living Well Outpatient care. We can sometimes care for people from other areas if, for example, close family and friends live in our catchment area. The referral process We want it to be as simple as possible for patients and their families to access our services. We welcome referrals for patients at all stages of their illness including early stages, so we can support them and their families and friends for as long as possible.
