Eligibility and the referral process

Eligibility for care from Trinity

We support people aged 18 or over who have a progressive, life-limiting condition and live in our catchment area. Our catchment area covers the whole of the London borough of Wandsworth, and parts of Lambeth, Merton, Westminster, Richmond upon Thames, Kensington and Chelsea and Hammersmith and Fulham.

You can see if you live within our catchment area by searching our map.

We can sometimes care for people from other areas if, for example, close family and friends live in our catchment area.

The referral process

We want it to be as simple as possible for patients and their families to access our services. We welcome referrals for patients at all stages of their illness including early stages, so we can support them and their families and friends for as long as possible.
