Eligibility and the referral process Eligibility for care from Trinity This page contains information for patients and potential patients, their families and friends. For referral information and a list of inclusion criteria for clinicians, please click here. We support people aged 18 or over who have a progressive, life-limiting condition and live in our catchment area. Our catchment area covers the whole of the London borough of Wandsworth, and parts of Lambeth, Merton, Westminster, Richmond upon Thames, Kensington and Chelsea and Hammersmith and Fulham. You can see if you live within our catchment area by searching our interactive map. The purple area denotes our catchment for inpatient, outpatient and community care. The smaller area denotes our community services catchment. We can sometimes care for people from other areas if, for example, close family and friends live in our catchment area. The referral process We want it to be as simple as possible for patients and their families to access our services. We welcome referrals for patients at all stages of their illness including early stages, so we can support them and their families and friends for as long as possible. Healthcare professionals can find out more about making a referral and download our referral form here.
