Wandsworth Care Coordination Service

If you live in Wandsworth, the award-winning Care Coordination Service can help you and your family by providing a one-stop-shop for all your care needs so you don’t need to go to different care providers separately.  

We arrange the services that you need while you are being cared for at home. The service is available to anyone aged 18 or over who is living in Wandsworth or who is registered with a Wandsworth GP. How the Wandsworth Care Coordination Service can help you

The Wandsworth Care Coordination Service is a one stop shop for all your care needs. We are here so you and your family don’t need to contact all of your different care providers separately.

We have been running the service since 2015. Our team of Care Coordinators, supported by experienced clinicians, can support you with:

  • Arranging and coordinating care agencies, community nurses, therapists and volunteer befrienders. We will review regularly to meet your needs.
  • Sourcing equipment like electric beds, commodes and hoists, depending on your needs. We will also arrange delivery and collection at home.
  • Providing information and advice about the services that are available to you and answering any queries and  concerns you may have about your condition.

This support can help you remain in your home, if that is what you wish, rather than be admitted to hospital.

Support in your home

Depending on your needs and condition, you may be eligible to receive support from the Wandsworth Care Coordination Service, provided by Royal Trinity Hospice carers. The dedicated team of carers can provide support with things like washing, dressing, loading the washing machine or preparing a simple meal. This support is provided short term, until agency care can be arranged.

Support for carers

From 10pm to 7am, Marie Curie Nurses and Healthcare Assistants are available so that the person caring for you can take a break while you are being looked after.

The service is open 8am-8pm seven days a week, including public holidays. If you need support outside of these hours, please phone 111.
