Moira and How's Story

Triage Clinical Nurse Specialist

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Triage Clinical Nurse Specialist within our First contact team. This post requires you to act as the first point of clinical contact for the patient, following patient referral. They will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.

Compassionate Neighbours is a social movement which helps to tackle the loneliness and isolation that many people face towards the end of life due to age or illness. We train local people to become Compassionate Neighbours and then introduce them to someone living close by who could do with some support.
